This update fixes an issue where some grave holograms could remain in the world even after the death chest no longer existed.
Changes:

• Fixed ghost holograms staying after graves expire or disappear.
• Improved hologram cleanup system.
• Grave data is now cleaned automatically if the death chest no longer exists.
• Holograms are now removed more reliably when a grave is deleted or manually broken by an admin.
• Added a new cleanup command for old stuck holograms.

After updating, restart your server and run /grave cleanup once in loaded areas where old ghost holograms may still exist.
